Case management for the aged, people
with a disability and their carers
We provide information, support and case management services for the frail aged, people with a disability and their carers living in the Belconnen region by:
- assessing their needs
- providing information on services
- assisting with coordination of services
Some funding may be available to assist in the provision of services or equipment that encourages and promotes independence.
We are a community-based organisation and these services are provided by professional and experienced workers who are flexible in responding to the individual needs of clients. Workers provide assistance across a wide range of support options to encourage and promote independence and assist people with complex needs.
